Choctaw County Birth and Death Records
Whether you are applying for a driver’s license, a new credit card, or any number of financial or legal documents, then you may have need to access a new copy of your birth certificate or another type of Vital Record. Birth certificates, like death certificates, marriage records and divorce records, are all considered Vital Records. If you need to obtain a copy of any of your Vital Records, or the Vital Records of anyone else, then the best place to begin is the website for the Alabama Department of Public Health.
This webpage is found at http://adph.org/vitalrecords/.
The ADPH even has a branch office in Chocktaw County. Its address is 1001 South Mulberry Avenue, and is located in Butler. They can be contacted by telephone at (205)459-4026.
If you are looking for the birth records of someone other than yourself, or for the death records of an individual, then there are a few things that you should be aware of. The first is that you must be able to demonstrate a certain level of relationship with the person that’s name on the Vital Record. Obviously, if it’s your record, then this won’t be a problem, but if it’s for a family member or a friend, be prepared to have identifying information with you.
The specific requirements for obtaining birth records can be found at http://adph.org/vitalrecords/index.asp?id=1559, and the requirements for death certificates can be found at http://adph.org/vitalrecords/index.asp?id=1560.
Finally, the first copy of each of these records will cost $15.00, and each subsequent copy will cost $6.00.
